About 401Health

401Health provides Rhode Islanders with an easy way to track symptoms, find vaccine records, and learn about COVID-19 testing, treatment, and vaccination.

All Rhode Islanders are encouraged to use the 401Health app and take an active role in slowing the spread of COVID-19 in Rhode Island.

If you submit this quick survey daily, it will help give us early indicators of COVID-19 spread in certain zip codes so we can respond by doing things like setting up testing sites in that area.

The "My COVID-19 Vaccine Record" feature has been modified so that the user can download and save their COVID-19 vaccine record in Apple Health and Wallet Apps.

401Health is a one-stop shop to get all the information you need about COVID-19 in Rhode Island.

The “My Symptom Diary” feature lets you tell us if you have any symptoms and only asks you to provide your zip code.

You can make a difference in the health of your loved ones and your community by helping us track the virus in our state.

Please see our Terms of Use ( ) & Privacy Policy ( ) for further information.

Great idea needs some improvements

I like that it keeps the vaccine record digitally. It would be helpful if you could also keep your testing record as well. As some of us have tested positive recently, that should be taken into consideration for some grace period to get the next booster.

By

Easy and Convenient

Like having my vaccine info in an app on my phone. No paper vaccine card needed. Lots of additional Covid info available as well

By

This app is just another typical government screw-up. .

Vaccination records are very inaccurate in 401Health. Do not rely on it. Refer to you card. Take it with you anb/or take a phone photo of it.

Doesn’t work

401Health won’t let you get passed the let’s go. Probably needs an update such a joke

By

Doesn’t work

401Health doesn’t work, it keeps getting stuck at “Let’s Go.”

By

Waste of time

Or you can just screen shot your vax card. What a waste.

By

Sad

Really disappointed in the premise of 401Health. It is un-American and a disregard for freedoms. We must do better.

By

Useless

Can’t get past the “Let’s go” screen.

COVID-19 Proof of Vaccination

Upload successful.

Battery killer app

I had this on my phone for a couple of months until I deleted it yesterday. I deleted it because it kills battery life. I have a phone that’s only a year old and after I downloaded 401Health , I would have to charge my phone a couple of times a day. Yesterday...I did use my phone a bit for creating videos in the morning and I was at 20% by 11am after a full charge overnight. So I deleted it after having enough of losing battery life like that.

The GPS was kind of inaccurate...it was in the ballpark of where I was. Since I don’t really go anywhere but home, work, the store and the gas station since Covid hit, I don’t really need the location diary

Location data is wrong, runs down battery fast

401Health is a good idea, but the location data is so wrong. I took a 30 minute walk yesterday and 401Health says I loitered on a corner that I wasn’t anywhere near for EIGHTEEN HOURS, and then that I went to a bar for four more hours. That’s..... so wrong. I would be hesitant to trust contact tracing information based on 401Health. I’m deleting and will just keep my own log in a notebook.

.... Tried this again after the update, had it installed for about 2 weeks this time. Location data is slightly better, but still placing me for long periods at spots I never visited, and missing places where I stopped for a while. Also, it runs down my battery noticeably faster than usual. I want to support contact tracing as much as possible, so I really wish this worked ... but I guess I will just continue my notebook log. Alas.

Tried it twice

I downloaded 401Health right at the beginning and within the first several days I noticed that the battery on my phone was close to depleted by the end of each day. Since I do not use the phone very often this was not typical. I thought about 401Health and deleted it. My battery levels then returned to my normal at the end of each day.
After another few days I decided to try again with more restricted permissions but the battery results were the same. Consequently I have again deleted 401Health .
I also did not see an accurate display on my travels with the GPS.
I may try again when a new version is released but if the battery usage problem continues that will be the end. By the way my battery is only one year old.

Location issues

I’m using the 2.2.05 version of 401Health. While it has improved a lot, there are still some serious issues still with locations. I’ll be home and it puts me 2 miles from my home! Or I’ll go somewhere and it puts me down the street. When I go to work, I’ve worked at 4 different locations according to 401Health ! The thing is 401Health asks you if you want location running. If I’m going to say yes, I want it running, and you are still killing my battery, have me where I’m truly at!!

Overall good: one suggestion

Overall 401Health seems easy to use and I appreciate the way it is designed to protect privacy. One suggestion I have is to add a place to just jot down a list of all the people we have interacted with each day right in 401Health . I know there has been mention of phones pinging each other later on, but this would be a simple and less invasive way to keep track at least in the meantime—sort of a running memo where every day I note who I saw as well as where I was, for example I was chatting with neighbors outside yesterday (6’ apart) which would not show up on a location map.

Covid diary the easy way but high battery usage

I fully support Covid mitigation efforts such as mask wearing and contact tracing. 401Health makes it so much easier to keep a diary of where I’ve been, especially if I forget to make a note that day. The only caveat I’ve noticed with using 401Health is the battery usage seems high. When I’m at home and through the night 401Health is always on top for highest battery usage. I wish there was a way to confirm your home or add your address so 401Health stops using the higher services such as location tracking and cell when in my home.

Still a big battery drain

I waiting awhile before downloading 401Health for data privacy reasons, and while I was encouraged to see the removal of location data after 20 day increments, it’s clear after only a few days of use that 401Health is a huge battery drain.
I typically get through the day with about 50% battery on my iPhone Xʀ after fairly typical use (web browsing, mail apps, music, etc) but I’m sitting here with 17% at 6pm. Yikes.
I want to help, but my phone won’t be much help if the battery is drained. Other apps on my phone have all day location access without the same drain, so it must be possible to tweak 401Health in some way so that it uses less energy.

Suggest adding a way to add a location entry manually

A lot of the stops I make aren’t long enough to register with 401Health. It would be good to have a way (a button, possibly)to mark a location and add notes. Also, I’d like to be able to add notes about who I interacted with before I leave a location. Having the entry not in the location log until you leave means you can’t enter people’s names while you are with them. Being able to make a manual entry would fix both problems.

Energy Hog

I downloaded 401Health on both my iPhone XR and my wife’s iPhone XS MAX the very first day the state of Rhode Island made it available but unfortunately it was killing our batteries by consuming more than 30% of the charge just while we were sleeping overnight. Not acceptable. We realize it was rushed to market in an effort to help control the pandemic and probably will be improved in subsequent versions. As it is now it’s not ready for prime time. I’ve also communicated with the state of Rhode Island and the developers following the link here in 401Health Store which I suggest you do too. They’re trying hard to control the virus and constructive criticism will be useful to them for the future.

Good but has issues

Overall 401Health is good however the collection of location data makes this a no-go for me. Really feel you should have used the API Apple has created gone that route. I understand the data location could be helpful but many people don’t feel comfortable with local, state, or federal government having that data. Apple’s API is very secure and you people can still get notified if they have been exposed. Would recommend switching to that.

Much improved version

Much improved version.
The notes make it much more useful. It’s really not 401Health ’s fault that GPS are not 100% accurate, so it’s good to be able to rectify address or enter the store’s name.
However you should really highlight the fact that phone needs to be at least 30% charged. On low battery, 401Health missed several of my trips.
Yes it’s a pain that 401Health uses a lot of battery but the purpose makes it worth it :-)

Crushcovidri app

I’ve accessed my location diary and it doesn’t seem to be totally accurate. Maybe it still needs to be tweaked a bit?
For instance I visited 3 addresses yesterday and only 2 showed up in my location diary. Also the 2 street names listed were correct but the numbers of the addresses were incorrect.

I can’t sign consent

When I’m going through the permissions, the page scrolls past the page to click on and goes right to a second page saying the I must accept the terms of use (which gets scrolled past without stopping).
I can’t add pictures here, but contact me if your like more info. Thanks!

Simple and safe

401Health is pretty straightforward and convenient to use. I hope other folks download it and it actually proves to be an effective tool.

Battery Drain

A lot of information in 401Health however appears to drain the battery on my iPhone 7 .

By

Easy way volunteer!

Seems like a simple easy way to help get things “normal” again!

Battery Drain

I really wanted to use this. My friend and myself experienced significant battery drain. 87% to 65% in 30 minutes on my iPhone 7. 25% of my battery usuage. Had to delete 401Health .

By

Great Idea and Easy to Use

Just downloaded and it’s easy as pie.
Get it.
Use it.

By

Sucked my battery dry

After using for 24 hours I noticed that my battery was running lower than normal. I checked my battery usage and found crush covid ri had used 95% of my energy.

By

Location not being accurately tracked

I was at a location for over a half hour and it was not recorded in my diary.

By

Beware!

Any information that is used becomes theirs. Make sure to read fine print. DELETE!

By

Don’t do it RI

Created by a company out of India that wants you to report your symptoms and track or personal info and location. That’s called HIPPA violation!

By

This is ridiculous

GINA REOPEN OUR STATE ALREADY. Get Rhode Island back to work, this is bs. If I wanted to live in communist China I’d just move there. Anyways, app is ok I guess

Can’t get past accepting terms on iPhone!

Just downloaded it today on iPhone. The terms have 4 pages. When I swipe to 4th page it doesn’t let me accept terms - page is there for a split second, I don’t have a chance to accept them, and it goes right to “you have to accept terms” with “go back” button. I click it and it takes me back to page 3 of terms. I swipe to the last page and- guess what - it takes me back to “you have to accept terms” - I haven’t been able to get past that loop. You need better QC!
