Consent form file not found

Such a shame, great idea but whoever manages their web infrastructure could use some basic monitoring skills. No consent form is a HUGE issue.

States with agreement of consent form but message occurs with not found. I ended up backing out and deleting app.

Keyboard Won’t Come Up

Great idea. Simple and quick, except when you click in the temperature box and your keyboard won’t come up. I tried clicking in multiple places multiple times, nothing. Tried on both iPad and iPhone with latest iOS. Same story trying to input my zip code, clicked “edit” and no keyboard, nothing, zip, nada. Tried clicking all over the page, no keyboard. I’m not going to spend hours troubleshooting what should be so simple. Sorry.

Update...After writing the above review and looking at others, I decided to try clicking other places on the screen to input my temp. I finally figured out it’s a rolling slider. I’m now onboard, but this interface needs to be more obvious. However, I still can’t input my zip code. I’m not sure how helpful the temp is without a zip code.

I was able to download COVIDControl and enter my data within seconds. If we want people share data it has to remain very simple like COVIDControl.
