My chart was closed because I attempted to put in a code to may code to many times?? need to get into my chart , what can I do
Entered misspelled password resulting in account lockout.
Unable to get on MyChart. States information incorrect . Info is correct .. nothing has changed. Name , address, birthdate , user and password has not changed
I have received 2 emails concerning upcoming appointment. Attempted to log in but was unsuccessful. Attempted FORGOT user name but did not get a reply. Attempted FORGOT password but did not get a reply. So I went in and set up new account for MY Chart thinking it would bring up my information, but it didn't. Would like to go to original account so I can send a message to my doctor
