this app sends notifications to my iPhone so that I fill out the required forms days before I arrive to my doctors appointment. Gives me the opportunity to use the screen reader to read the questions and I can sign my forms before I get to the office while allowing me to pay my co-pays.

Being able to schedule appointments and fill out forms online is so convenient! Not having to arrive early to my appointments to take care of paperwork takes customer service to the next level.

MyCarePlan allows me to quickly fill out all forms prior to my appointment with my healthcare provider digitally on my phone. When I arrive for my appointment, I’m seen by the doctor on time. Love this convenient option!

My doc started using this so I need to sign forms. I’m shown a nice list of forms. But when I read and sign any of them I get an http failure (500 Internal Server Error).

And this is just an iPhone app so anyone on an ipad with keyboard will have to turn your head 90 degrees to try to use it. It’s 2019: apps like that should be removed from MyCarePlan Store.

I think I need to find a new doc if he’s going to insist on using this junk.

I am so glad my doctor’s office has MyCarePlan! It was so easy to use. I booked my appointment via MyCarePlan and also filled out my forms prior to my arrival. It is so convenient!

MyCarePlan is great. So easy to use. You can schedule appointments on your phone & fill out all your pre-appointment forms digitally, right from your phone.

These forms were completed a month ago but today I had to reload the information. I’m guessing the data was lost? Or did I choose the wrong app the first time? Two apps with very similar names are in MyCarePlan store! Seriously? Check the market before naming then posting an app. Geez!

There is no option for adding secondary insurance provider information.

No option to review forms after submission. update prescriptions, medical history, or add physicians as care needs change. Wow!

Absolutely a junk app that lacks minimal functionality.

Why have MyCarePlan if you can’t change your password? And they don’t help you! See just trying to leave this message and you have to leave a nickname and if it’s taken, try again. You can’t leave it blank! Stupid.

MyCarePlan is awful. It takes more time to fill out forms than with pen and paper. MyCarePlan then doesn’t allow you to save with vague errors with no directions on how to fix them. It’s clearly a wrapper around a web app so it feels like a poorly written and designed web app.

Very intuitive and functional

I love the this app app. It’s a time saver. And, it really helps me to take control of my health. I never miss a task or an appointment again.

Great App!! Very helpful on having all my Tasks completed before coming to an appointment.

You can see each other but no sound. MyCarePlan has NOT asked to use microphone, so basically MyCarePlan is useless

My care plan is ridiculous. After putting in all my info it does not allow you to list your providers your doctors or anything that is not in their list. A total waste of time

No biometric Authentication. You have to put your password in every time. Couldn’t get LastPass to work either. So I had to manually add.

Need to use this for my telemedicine with doctor. Glitchy...can’t open visits for different family members with same office. Tech people not helpful.

Doesn’t let you sign in. I have everything I need on the screen but when you press “Go” nothing happens...

The system does not work consistently as it is intended too.

Login works on the computer, not on the phone app.

Patients and providers can finally team up to improve care.

Sometimes it works sometimes not. One time I was able to use it. Now Doctor’s office tells me there is info to check on MyCarePlan . I go back - nothing. All blank...

App is so simple to use ... great idea
