Forgot password message is not sending me an email. It does not recognize my account. I've had it for years and have a lot of points in there. I went to Costa Vida tonight and noticed you cannot scan your receipt anymore. I tried logging in, but it won't let me. Help.

The receipt I received will not allow me to scan the code as it is not clear . . can I manually enter the Survey Code? The survey code is 824913000005211
