The Tria Health app will then allow the member to access their personalized care plan developed with their Tria pharmacist at any time.  You’ve taken the first step and worked with a Tria Health pharmacist to develop a personalized care plan.  In order to access TriaHealth , members will have to enroll with Tria Health and complete an initial consultation with their Tria Health pharmacist.  Tria Health’s medication reminders allow you to track your medication in any form, at any time of day and ensure you’re aware of when you need a refill.  Tria Health’s new mobile app will enhance your ability to interact with Tria Health and empower you when it comes to managing your health.  Log In Using Your Tria Health Patient Portal Account: myportal. this app.  Now there’s an easy way to access everything you discussed with your pharmacist from medication recommendations, preventative services and more!  Medication regimens can vary in complexity and we know you need more than just a simple alarm to manage it all.  The planner tool allows for greater freedom in customizing your medication schedule.  You’ll never have to worry about forgetting what you wanted to discuss with your doctor, you’ll have it ready to go in the palm of your hand.
