this app® partners with public health heroes to make it easier than ever for you to access personal and family immunization records directly from Idaho Department of Health and Welfare, Minnesota Department of Health, New Jersey Department of Health, and Utah Department of Health. NOTE: You must have a valid email address or phone number on file with your state's immunization registry in order to access personal or family immunization records using Docket. Idaho, New Jersey and Utah residents can access a SMART Health Card vaccine credential QR code to facilitate travel to places with COVID-19 restrictions. Please consult with your health department if you have trouble locating your records using this service. Track upcoming shots, review past immunizations, and share your official immunization reports. Refer to the pinned tweet on our Twitter @DocketHealthApp for step-by-step troubleshooting instructions. Please stay tuned for announcements from other states.
