A SMART Health Card is a digital or printed version of your vaccination history or test results, shared through QR Codes and issued by supported states, pharmacies, and providers.
The SMART Health Card Verifier app allows you to scan a SMART Health Card QR code to quickly verify an individual’s COVID-19 test or vaccination credential.
If you have not been issued a SMART Health Card yet, please watch for news from your state or provider as many will be adding the SMART Health Card option in the coming months.
SMART Health Card Verifier is a service brought to you by The Commons Project Foundation.
Businesses like bars, restaurants, schools, and live event venues can use this free app to validate SMART Health Cards upon entry.
NOTE: The Verifier app only scans SMART Health Cards.
To be recognized as part of the CommonTrust Network, issuers must be confirmed as trusted entities and must issue verifiable credentials.
It does not scan paper CDC cards.
