1. On your homescreen, Tap and hold Iconify - Custom Icons Themes until it starts shaking.
2. Once it starts to shake, you'll see an X Mark at the top of the app icon.
3. Click on that X to delete the Iconify - Custom Icons Themes app from your phone.

Method 2:
